📝 How to Fill Out the Civil Service Exam Form

Follow these steps carefully when filling out CS Form No. 100:

  1. Read the instructions at the back page before starting.
  2. Fill in all required fields. Write “N/A” for items not applicable.
  3. Indicate the examination applied for (Professional or Subprofessional).
  4. Provide your personal details: full name, date of birth, sex, civil status, and contact information.
  5. Attach your recent ID photo following CSC photo specifications.
  6. Do not fill out the shaded portions—these are for CSC processors only.
  7. Sign the form in the presence of the administering officer.

Tip: Use black ballpen only. Avoid erasures or alterations to prevent rejection.

Along with the accomplished form, applicants must submit the following:

  • Four (4) identical ID photos (Philippine passport size, with white background)
  • One (1) valid ID (not expired) such as:
    • Driver’s License
    • Passport
    • SSS/GSIS/PRC/PhilHealth/Postal/Voter’s ID
    • Company or School ID
    • NBI or Police Clearance
  • Birth Certificate (if date of birth not indicated on the ID)
  • Examination fee:
    • ₱500 for CSE-PPT
    • ₱680 for COMEX
  • For dual citizens: Certificate of Retention/Re-acquisition of Philippine Citizenship

📍 Where to File the Civil Service Exam Form

Applicants must personally file their application at the nearest CSC Regional Office or Field Office. Applications submitted by mail or courier are not accepted.

After submission, successful applicants will receive a Notice of School Assignment (NOSA) through:
